Your tattoo artist isnt going to be able to give you any help other than to run through how you cared for it, try and guess why it happened, and then send you on your way.

It looks kind of milky but ive only ever seen it when its been used as the skin of a portrait of a person.

Its hardly noticeable, best that you ask your artist if thats a good idea or not.

In my experience around the 10 year mark detailed tattoos, especially of peoples faces etc.. start to look blury and the eyes lose some fine detailing.

If you have dry flakey skin, use warm soapy water to remove it. Then use a good Moisturiser.

I wouldnt recommended scratching it like crazy. In my opinion if it feels GOOD your scratching too hard. ...
